The road to wellness is paved with prevention. One of the best ways you can stay on path to good health is by scheduling your Annual Wellness Visit (AWV). The AWV is a benefit of your Medicare plan, so this visit comes at no extra cost to you. If you and your doctor discuss other conditions or you are asked to complete additional screenings, you may be responsible for a co-pay or co-insurance.

Here’s how an AWV can help you make this year the year of YOU:

Communication Is Key

An AWV is not an exam or a physical. It’s a conversation. It’s your chance to talk to your care team about how you’re doing on your journey to wellness. During your AWV, you’ll have plenty of time to ask whatever questions you like about your health and wellness goals.

Before Your Visit

Please click the link below and take a few minutes to answer the questions before your visit. This will give you some time to collect your thoughts and prepare for the conversation, so you won’t forget anything important during your Annual Wellness Visit.

During Your Visit

  • Your care team will ask you to fill out a questionnaire, also called a “Health Risk Assessment.” This will give your care team a snapshot of your current health.
  • Remember to share any current conditions or medications, as well as any other medical visits you may already have planned.
  • Your care team will assess your memory and thinking to help maintain your brain health.
  • Before you leave, you and your care team will discuss a care plan for the year ahead, including a schedule of other preventive checks you might need throughout the year

After Your Visit

  1. Follow the plan you create with your care team.
  2. Schedule follow-up appointments you might need for 2022 screenings.
  3. Be proactive so you can achieve your wellness goals for the year.
